Back to School Questionnaire

Building relationships is the single most important thing about teaching. It can take several months to get to know your students. This student questionnaire helps you learn more about them and use as conversation pieces in building your relationship with them. Plus, who doesn't love to talk about themselves? Most kids want to share details about their lives. This gives them that opportunity. You could even use it for a game to help students form new friendships. 

The parent questionnaire is a bonus! You have to make those connections with parents right away. They love bragging on their kids. They love it even more when you acknowledge that they know their babies better than anyone else does. Ask parents to fill this paper out on Back to School Night. It is a great way to get insight on your students from a special perspective!
